•Individuals who can independently perform activities of daily living (ADLs).
•Individuals transitioning from homelessness.
•Individuals needing stable, supportive housing.
•Individuals motivated to maintain independent living.
•Individuals seeking independent living while also wanting guidance and assistance with access to community resources.
Submit a Resident Referral
All referrals are reviewed promptly.
A member of our team will contact the
referring agency within 1-2 business days regarding eligibility and next steps.
